Toronto Coroner's Report on increasing bicycle safety. The Coroner's report is famous for it's conclusion, "The concept of motorized vehicles yielding to non-motorized vehicles, who in turn must yield to pedestrians seems to be a common sense rule which should be accepted by all road users." But the report also makes a number of useful recommendations involving education, enforcement, identification and repair of problem areas, and data collection/review.
